The years after (2) Questions and uncertainty remain; theories about conspiracies Workers and residents demand medical examination 1998: A trace of uranium is found in Hangar 8 AMC study Parliamentary inquiry Several employers decide to offer a medical examination to their employees Examination is also offered to residents and to other assistance workers
MOVB Individual examination Epidemiological study 1999: MOVB: ‘Medisch Onderzoek Vliegramp Bijlmermeer’ Project management: KLM Health Services Individual examination Epidemiological study
Individual examination Goals: Determine health problems Determine health problems related to the disaster Reduce anxiety When needed treatment advice Questionnaires, lab testing, physical examination and counseling Psychological symptoms Somatic symptoms Follow up consultation: results When needed: aftercare
Epidemiological study Two protocols Assistance workers Residents Strictly defined Exposed and Reference groups 70 % response required Questionnaires, blood, saliva and urine samples
Police officers: prevalence of ICPC (1+) symptoms * = p < 0.05 (OR, adjusted)
Symptoms attributed to disaster (% of workers with symptoms)
Summary Exposed workers reported considerably more physical symptoms, over a broad spectrum. There were no differences in laboratory variables between exposed and reference workers. We found only marginal relationships between exposure level and symptoms. Many workers saw a (not very strong) relationship between their symptoms and the disaster
‘a normal reaction to abnormal circumstances’ Conclusions The elevated incidence of Medically Unexplained Physical Symptoms is in accordance with literature on post-disaster health (Allthough the literature is mainly on shorter term effects, and on disaster area residents) And can therefore be considered to be ‘a normal reaction to abnormal circumstances’ Abnormal circumstances = disaster and aftermath
+ and – of medical examination (individual) Reassurance Tailored treatment for PTSD, MUPS Ritual closing - Medicalisation ? Fault positives Use of medical resouces
+ and – of epidemiological study Answer to speculative hypotheses Answer to other hypotheses Scientific information -> relevant for future disasters - Excessive attention Expectations not met
(Cambodja, Balkan) veteranen
Waarom gezondheidsonderzoek Optimalisatie individuele zorg Optimalisatie publieke gezondheid Inzicht in blootstellingssituatie Inzicht gezondheidssituatie: aard, aantal, ernst, verloop Signaleren risicogroepen Anticiperen op hulpvraag, effecten van interventies Zorgbeleid Nazorgvoorzieningen, communicatie Evaluatie: aanpassen procedures, richtlijnen, etc Wetenschappelijk: Kennis- en leerdoel Maatschappelijk / politiek / bestuurlijk
Typen gezondheidsonderzoek Individueel Groepen Monitoring
