Jejunal aspiration “Gold Standard” - Quantitative detection of excessive bacterial concentration in jenunal aspirate ( < endoscopy or fluoroscopy with jejunal intubation) - > 105 bacteria per ml intestinal fluid (nl < 104 bacteria/ml) includes gram postive flora (< upper respiratory flora) > colonic bacteria ! Limitations: - not all species can be identified. ( > 500 sp) / not always representative - Fals positive results due to contamination of oropharyngeal flora - Fals negative results due to patchy / difficult to access localisation (predisposing abnormality) 34
