For neurological diseases as CVA or dementia, the use of Clinical Pathways is rather complicated. This Clinical Pathway for dementia brings a solution for some methodological problems and has the intention to ensure the continuation of care between home, ambulatory care, hospital, and nursing home. As the number of demented persons will increase, the development of a structured approach with respect for the individual is very important.

The introduction of the pathway in our hospital leads to a higher recognition of patient with dementia. Using the clinical pathway in an ambulatory setting is efficient and attractive. The use of a web based database system for follow up, enables the transmural aspect of the care. The development of Clinical Pathways in dementia should be encouraged.
