1 Casus 1 Anamnese Kelim B. Is een jongen van 10 jaar. Wegens rugpijn bezocht hij samen met zijn moeder de huisarts. De huisarts stuurt hem naar de kinesitherapeut wegens rugpijn. Bij lichamelijk onderzoek was er een lichte paravertebrale hypertonie. Er was sprake van een licht versterkte thoracale kyfose, zonder aanwijzingen voor scoliose. Hij heeft tevens een navicular drop en genu valgum, links meer dan rechts. De pijn in de rug treedt vrnl op bij lang recht staan. Kelim heeft ook astma waarvoor hij oxis en pulmicort neemt. Zijn moeder heeft een briefje aan school bezorgd zodat Kelim niet hoeft mee te doen met de sport- en turnlessen.

12 Casus 2 -Mary weighs 74 kg, and her height is151 cm, with a body mass index of 32 kg/m 2. Blood pressure is 130/75 mmHg, pulse 95 beats per minute, respiratory rate 24 breaths per minute. -Mary J. is a 12-year-old obese girl. Her parents encouraged her to join a running club because they are concerned about her weight. She participates in a start-to- run program of the club since 6 weeks. This program aims to run three times per week with a group of children. Mary participates 1 or 2 times per week. -Since two weeks she complains about pain in the left knee. She also complains of fatigue after running. The energy drink does not help; it only makes her thirstier and makes her have to get up during the night to urinate.

13 Casus 2 Physical examination -The patient had prominent acanthosis nigricans around her neck and, more subtle, over the flexural creases of her antecubital fossae. On the knees, there was no evidence of erythema, hyperpigmentation, or open wounds. All ligamentous and meniscal structures, including the anterior and posterior cruciate and medial and lateral ligaments, were intact with provocative stress testing. There was no discernable area of tenderness in either knee. She had equal and full sensation and strength bilaterally with palpable pulses in her lower extremities.

14 Casus 2 Physical examination -Examination revealed that she held her left hip in marked external rotation compared to her right. She was unable to actively fully internally rotate or flex her left hip. Her right side had a full range-of-motion. In contrast to the right hip, passive internal rotation of her left hip was significantly limited. She reported left groin pain with forceful internal rotation of her left hip. -Her abdomen was obese and soft with mild epigastric tenderness. She had Tanner III development. -Weight bearing on the affected lower extremity reproduced her pain. The left leg was approximately 1 centimeter shorter than her right. The rest of her examination was normal.

16 Casus 2 Additional info -RX: Anteroposterior and frog-leg lateral views of the pelvis of a 12- year-old girl with a chief complaint of left knee pain for 3 weeks. Note the widened and irregular physis on the anteroposterior view and the inferior displacement of the capital femoral epiphysis on the frog-leg lateral image. -> Epifysiolysis Capitis Femoris -Lab results: Lab evaluation revealed severe diabetes type 2 with diabetic ketoacidosis (DKA) and dehydration. -Her family history was significant for type 2 diabetes in her father, with onset in his 30’s.
