Slide 3 The benefit of platelet aggregation inhibitors across a wide spectrum of patients is supported by the findings of the Antiplatelet Trialists’ Collaboration.1 They conducted a meta-analysis of 142 trials including more than 73,000 high-risk patients. This meta-analysis showed that platelet aggregation inhibitors reduce the risk of the composite outcome of ischemic stroke, myocardial infarction (MI), or vascular death by 25%. The relative odds reduction was consistent across patients with a wide range of clinical manifestations of atherosclerosis: platelet aggregation inhibitors reduced the risk of vascular events by 22% in patients with prior transient ischemic attack (TIA), 29% in patients with acute MI, and 25% in patients with prior MI. The observed benefit was also consistent across subsets of patients at various levels of risk within specific clinical disorders (for example, hypertensive versus normotensive patients). 1. The rationale for the CAPRIE study was based on the fact that patients with a wide spectrum of atherosclerotic disease, including disease of the cerebrovascular, coronary, and peripheral arteries, are at risk of major ischemic events such as myocardial infarction, ischemic stroke, and even vascular death. Atherosclerosis may be complicated by a thrombosis—a process called atherothrombosis. The atherothrombotic process is similar, regardless of the initial clinical manifestation of atherosclerotic disease. Platelet aggregation inhibitors are effective in reducing major ischemic vascular events in patients with atherosclerosis regardless of the initial manifestation of their disease (cerebrovascular, coronary, or peripheral arterial disease).2 Thus clopidogrel is expected to benefit the entire spectrum of atherosclerotic patients. 1. In deze dia wordt de distributie van de symptomatische atherotrombotische complicaties bij de CAPRIE-populatie weergegeven. Bij meer dan 25% van de patiënten werd symptomatisch atherotrombotisch vaatlijden in meerdere vaatbedden aangetroffen. Thus the safety of clopidogrel was a major consideration in the CAPRIE trial. Aspirin was associated with statistically significantly more gastrointestinal hemorrhages (255) than was clopidogrel (191).1 There was also a trend to more intracranial hemorrhages and hemorrhagic deaths in the aspirin group than in the clopidogrel group. The risk of clinically relevant GI bleeding with clopidogrel is lower than with aspirin. Furthermore, this difference in favor of clopidogrel is likely to be underestimated in CAPRIE due to study exclusion criteria. There were also 30% more hospitalizations due to gastrointestinal events in the aspirin group (104) than in the clopidogrel group (71).2 1. CAPRIE Steering Committee.
