2 2 Geen Nederlandse kosten effectivteitsanalyse  Geen studie volgens de richtlijnen… –Richtlijnen kosten effectiviteit in Nederland goed ontwikkeld –Vooral bij geneesmiddelen (CvZ)  Eerst studie start dit jaar –Rotterdam patiënt doen waarschijnlijk mee –Resultaten over 3 jaar

3 3 Gebrek aan economisch bewijs geen uitzondering…  Welsh & Farrington, 2000 –[…] little is known about the economic efficiency of crime prevention strategies. –A review of the literature revealed only seven [7] published studies….  McDougall et al, 2003 –A systematic review …. only nine [9] published studies  Soeteman & Busschbach, 2008 –12 studies

4 4 Economische studies in the USA  Steve Aos et al, 1998 and 2004 –Model studies  Direct costs (2004) –Costs about $ 5,681 per participant –Reduces level of felony by 44% –Saves taxpayers $ 14,996 per participant in future criminal justice system –For taxpayers a net gain of $ 9.316 for every highrisk juvenile  Indirect costs (1998) –An additional $13,982 in future costs paid by crime victims –Taxpayer and crime victim benefits combined net gain of $21,863 per participant

5 5 Hoe waarschijnlijk zijn deze positieve resultaten buiten de VS?  Verschillen –Kosten Nederlandse arbeid duurder –Verschillen in justitieel systeem –Verschillen in de deelnemers Ook civiele deelnemers  Algemene ‘divers’ in kosten effectiviteit –Werkt het? –Is het veel duurder dan het alternatief? –Nu of in de toekomst? –Compliances (therapietrouw) –Adherence (behandelingintegriteit)

6 De uitzichten zijn gunstig  Evidence based –Het is aannemelijk dat het werkt….  Relatief goedkoop –€ 300 per dag (JJI) versus € 12.500 voor MST  MST werkt nu én in de toekomst…  Therapietrouw onderdeel therapie  Behandelingintegriteit onderdeel therapie

7 7 Uitdagingen  Wetenschappelijke basis jeugdzorg is zwak –Vergeleken met somatische zorg  In wetenschap jeugdzorg nauwelijks oog voor maatschappelijk uitkomsten –Wetenschappers focussen sterk op ethologie (oorzaak) –“Intermediate outcomes” bijvoorbeeld gezinscohesie –Kosten en recidive meestal afwezig  ‘Genoegdoening’ MST niet duidelijk –Voordelen moeten gebrek aan genoegdoening compenseren

8 8 We are on the right track…  Rand Europe (2006) –Interventions to reduce anti-social behaviour and crime –A review of effectiveness and costs  Conclusion –The relative absence of rigorous programme evaluations in Europe means that there is insufficient data to allow robust analysis of the effectiveness of many interventions to reduce anti-social behaviour. – However, there are some interesting evaluation initiatives currently underway in the Netherlands, such as the construction of cost-benefit models and careful tracking of new programmes.
