5 Inleiding Glomerulonefritis Casus 4
Patiënt, 54 jr. oud gekend voor SLE heeft veel eiwitverlies in de urine en de nierfunctie gaat achteruit. Bij immunofluorescentie onderzoek van het nierbiopt zijn er overwegend mesangiale en hier en daar deposities t.h.v. de capillaire wand van IgA, IgG, C1q, C3, kappa en lambda zichtbaar. IgG is moeilijk interpreteerbaar. Alles wijst op een ‘full house’ wat verwacht wordt bij SLE. Conclusie: Lupusnefritis klasse III tot IV 4

19 Conclusie Naar aanleiding van de gedeeltelijke automatisatie van immunofluorescentie op paraffinecoupes en vriescoupes van nierbiopten met de Benchmark XT vertoonde de FITC kleuring op de vriescoupes het beste resultaat. Dit in tegenstelling tot de kleuring van de paraffinecoupes waarbij het weefsel volledig aankleurde of helemaal niet. Om een geautomatiseerde immunofluorescentie op paraffinecoupes uit te voeren moet er een geschikt protocol aanwezig zijn in de software van de Benchmark XT. Aanpassingen aan bestaande protocols leverde niets op. De poging om te automatiseren is dus niet compleet geslaagd. 18
