4 CRHT The main aim of the service is to help you to manage and resolve your crisis through assessment and treatment in your own home environment as an alternative to admission to hospital, The team supports people where facilitated discharge is indicated as part of a discharge meeting involving the person and their carer, enabling them to transition back into the community to continue their recovery at home. People can contact the CRHT directly where this is part of their Crisis Contingency plan agreed with their Community Care Coordinator. Carers can also contact CRHTs. Generic advice will be offered but consent of the people under the care of the service will be required to engage in a face to face assessment.

9 Discussie We weten uit de literatuur dat het gebruik van signaleringsplannen effectief is. Zelfmanagementstrategieen worden breed ingezet binnen Dimence. Binnen dimence heeft de meerderheid van de patienten een al dan niet actueel signaleringsplan Wat kunnen we met de resultaten van dit onderzoek binnen Dimence?
