5 Nadelen neoadjuvante chemotherapie
tumor ongevoelig  uitstel lokale behandeling echter: ongevoeligheid zeldzaam (RR 90%, CR 3-10%; 12-25%, progressie 3-7%) toename chir. en RT gerelateerde complicaties geen accurate “base line” histopath. evaluatie echter: klierstatus na neoadj. chemotherapie verschaft belangrijke progn. informatie (mogelijk belangrijker dan progn. informatie van klierstatus vóór neoadj. chemotherapie)

6 Vragen Indicaties? Schema? AC, EC, FAC, FEC, taxanen (simultaan?, cross-over?), duur, ook postop.? Plaats neoadj. hormonale behandeling? Kan effect neoadj. chemotherapie voorspeld worden? Rol genetische profilering voor en na chemotherapie? Plaats MRI?

7 Vragen 7. Uitgebreidheid excisie primaire tumor na neoadj. chemotherapie (gebaseerd op situatie voor neoadj. therapie of na neoadj. therapie)? 8. Regionale therapie (chirurgie, RT) gebaseerd op klierstatus voor of na neoadj. chemotherapie? (SN-biopsie voor of na neoadj. chemotherapie?; neoadj. chemotherapie steriliseert in 20% pos. axillaire klieren) 9. Bij axillaire dissectie ook level III verwijderen?
