Verhoging van het serumbilirubine waardoor witte sclerae en huid Jaundice and asymptomatic hyperbilirubinemia are common clinical problems that can be caused by a variety of disorders, including bilirubin overproduction, impaired bilirubin conjugation, biliary obstruction, and hepatic inflammation.

6 CASUS 1 Anamnese casus: 6 weken in Uganda verbleven vanwege ontwikkelingshulp, voornamelijk in de stad, maar ook uitstap naar bosrijkgebied en lake bunyonyi Aldaar 1 week voor vertrek met gastro enteritis opgenomen in kliniek voor rehydratie en antibiotica, kuur overigens niet afgemaakt Sinds een week na terugkomst algehele malaise op en af Sinds 3 weken op en af koorts tot 39.5 Sinds 3 weken aanvallen van hoofdpijn, lijkt wel migraine, laatste week enkele dagen achtereen

Niet acuut ziek, RR 90/60, 80/min, T 38.3 Geen lymfadenopathie, geen hepatosplenomegalie, geen huidafwijkingen Hepatosplenomegalie en lymfadenopathie past meer bij hematologische ziekte Wilson’s

Hb 8.0, T 430, L 12, diff: lymfocyten 8 met atypische kenmerken ALAT 117, ASAT 280, LDH 350 Wat is je diagnose? Verhoogd bilirubine geisoleerd en mn ongeconjugeerde of indirecte geeft pre-hepatische oorzaak aan DD overproductie van bilirubine (hemolyse) of verstoring in de bilirubine conjugatie Hemolyse; verhoogd LDh, laag haptoglobine, laag hb, fragmentocyten, hemosiderine in de urine

13 CASUS 1 Nadere anamnese.. - Patiënte doet de bijbelschool, werkte voor de kerk in Uganda - Heeft ugandees vriendje en onveilig sexueel contact! - HIV dus ook getest (ook zonder het verhaal van ugandees vriendje misschien de moeite waard dit te testen) NB feces PCR positief voor shigella De acticiteit van het enzym UDP-glucuronyltransferase in de lever evrlaagd Crigler-najer syndrome type volledige afwezigheid van de conjugatie , type 2 10% minder, bili is hoger dan bij gilbert

16 CASUS 2 Overwegingen Mammacarcinoom
Koorts na punctie, bacteriemie/sepsis Andere reden van lymfomen infectieus (viraal/bacterieel) Uitzaaing andere solide tumor/hematologisch NB Pijnlijke klier!! Steatose door alcohol vermindert de oxidatie van vetzuren in de levercel, dus ook vaker hoge triglyceriden, deze hopen zich op in de levercel Meestal asymptomatisch, soms iets vergrote gladde lever Hepatitis: icterische en niet icterische, bij de icterische vaak pijn RBB koorts en icterus DD cholangitis bij galsteenpathologie. Slcehte prognose behandeling met prednison helpt! Niet icterisch geen duidelijke symptomen behalve afwijkend lab. Bijalcoholische hepatitis is IgA verhoogd! Pas lever transplantatie na 6 maanden geen alcohol

We recommend cholecystectomy in patients who have had an episode of pancreatitis and have biliary sludge because of the high risk of recurrence [32,36]. We zien het steeds vaker. 25% van de engelse bloeddonoren bleek antistoffen te hebben. klinische beeld is conform andere virale hepatitis. De misselijkheid, braken, verminderde eetlust, vermoeidheid, koorts en buikpijn.
