1 Late gevolgen kindermishandeling

Hoge maatschappelijke kosten (Felitti e.a.; walker e.a.; Tang e.a.; Zanari e.a.) Voor Nederland: 2 – 3 miljard per jaar (Rensen n.a.v. ACE studies)

10 Maatschappelijk opvang 56.000
Opvang cijfers 2007 : Vrouwenopvang Maatschappelijk opvang Kinderen t.m 17 jaar 8000 Waarvan 1800 alleen Jongeren jaar 7200

11 Clienten in Opvang: elkaar versterkend problemen:

13 Veelheid problemen: verschillende levensgebieden

Mishandeling: Psychisch (ouders) 11 % Fysiek (ouders) 28 % Seksueel (ieder) 22 % Verwaarlozing : Emotioneel % Fysiek %

17 ACE Studies 2: Disfunctioneren thuissituatie: Alcohol / drugs % Verlies ouder (onder 13) 23 % Depressie of psychische ziekte 17 % Geweld naar moeder 13 % Ouder in gevangenis 6 %

18 Adverse Childhood Experiences Score
Number of categories (not events) is summed… ACE Score Prevalence 0 33% 1 25% 2 15% % 4 6% 5 or more %* Two out of three experienced at least one category of ACE. If any one ACE is present, there is an 87% chance at least one other category of ACE is present. * Women are 50% more likely than men to have a Score >5. 7

19 Childhood Experiences Underlie Chronic Depression
Mental Health Childhood Experiences Underlie Chronic Depression Understanding that not every depressed person acknowledges that they are depressed, we’re probably understating the power of this relationship.

20 Childhood Experiences Underlie Suicide Attempts
Mental Health Childhood Experiences Underlie Suicide Attempts 4+ 3 At ACE Score 6 and higher, there is a fold increase in the likelihood of later suicide attempts. These are extraordinary numbers. Epidemiologists at the CDC tell me they are likely to see numbers of this magnitude once in a career. 2 1

21 ACE Score and Hallucinations
Mental Health ACE Score and Hallucinations Abused Alcohol or Drugs Ever Hallucinated* (%) Here is another measure of psychological outcomes: the making of madness. It would appear that psychosis has also deep roots, often extending back into childhood. ACE Score *Adjusted for age, sex, race, and education.

22 Adverse Childhood Experiences vs. Smoking as an Adult
Health Risks Adverse Childhood Experiences vs. Smoking as an Adult % This is the experience in San Diego, where public health pressure against smoking is unusually strong and intrusive. Yet, it is noteworthy how, in spite of major public health pressure against smoking, there has been no net decrease in the past 13 years. Could it be that something is being missed? Is smoking the core problem or the marker for something deeper? The psychoactive benefits of nicotine are profound in terms of anti-anxiety, anti-depressant, anger-suppressant, and appetite-suppressant activity. Even though the Indians knew nothing about nicotinic and muscarinic receptors in the CNS, they did know to smoke tobacco leaves and not oak leaves in a peace pipe. Does any of this matter, or do we blame it all on Marlboro billboards? p< .001

23 Childhood Experiences vs. Adult Alcoholism
Health Risks Childhood Experiences vs. Adult Alcoholism 4+ 3 2 This is self-acknpwledged alcoholism. Would we accept as an explanation that someone is an alcoholic because there is a liquor store in the neighborhood? Yet, we accept McDonalds as the explanation for childhood obesity, and drug dealers as the explanation for drug use. 1

24 ACE Score vs Injection Drug Use
Health Risks ACE Score vs Injection Drug Use ACE Score 6 becomes an exponential change to 46-fold increase, 4,600% increase. p<0.001

25 Childhood Experiences Underlie Later Being Raped
Well-being 4+ 3 2 This was a real surprise. What are the linking mechanisms that make this a reality? 1

26 ACE Score and Indicators of Impaired Worker Performance
Social function ACE Score and Indicators of Impaired Worker Performance 5 10 15 20 25 1 2 3 4 or more ACE Score Impaired Performance (%) Prevalence of Here’s how this plays out in the workplace, decades later. Absenteeism Serious Serious ( > 2 days/month) Financial Problems Performing job Problems

27 The ACE Score and the Prevalence of Liver Disease (Hepatitis/Jaundice)
Biomedical Disease The ACE Score and the Prevalence of Liver Disease (Hepatitis/Jaundice) Percent (%) The relationship to biomedical disease makes sense, once one routinely looks back in time. AACE CE Score ACE Score

28 ACE Score vs. COPD Biomedical Disease 0 1 2 3 4
But not everyone with COPD is a smoker or asthmatic. How does that subset originate?

29 ACEs Increase Likelihood of Heart Disease*
Emotional abuse 1.7x Physical abuse 1.5x Sexual abuse 1.4x Domestic violence Mental illness Substance abuse 1.3x Household criminal 1.7x Emotional neglect Physical neglect * After correction for age, race, education, and conventional risk factors like smoking and diabetes. Circulation , Sept 2004 . Biomedical disease Since the Framingham Study, we all think of risk factors when it comes to coronary disease. But conventional risk factors don’t explain 15% of cases. What causes THEM? It recent years, it has become evident that with major chronic stress there comes hypercortisolemia, and sometimes dysregulation of the Hypothalamic-Pituitary-Adrenal axis with the release of pro-inflammatory chemicals that cause organ scarring as the result of the small vessel inflmammation they induce. THIS appears to be the issue in a minority of cases of coronary and pulmonary disease.

30 Notice how far downstream we enter the picture - after disease has occurred. And yet, what we are discussing now enables us not only to identify risk factors - but to identify the DETERMINANTS of risk factors.

31 What are conventionally viewed as Public Health problems are often personal solutions to long-concealed adverse childhood experiences. “It’s hard to give up something that almost works.” A Public Health Paradox This realization was slow in developing, but its implications are huge in terms of why so many public health problems are so difficult to approach.

32 Return on investement Felitti:

33 Return on investment Artikel Reformatorisch dagblad 2007 (Rensen):
