3 Achtergrond Persoonlijke factoren worden niet consequent, systematisch en gestandaardiseerd geregistreerd Persoonlijke factoren zijn belangrijk voor continuïteit van zorg¹ Overdracht naar collega huisartsen Personal factors are very important in everyday general practice. Demographic variables, socio-economic factors, biographic data, lifestyle factors, life events and personal work related factors are the main contextual (personal) factors which form the background of the daily contact between patient and the health care professional. In the past, when paper records were used to register patient data, important personal factors were written on the main page of the medical records. This gave general practitioners (GP’s) a clear view of the personal factors of a patient. With the introduction of the Electronic Medical Records (EMR), this overview page disappeared. Only a few personal factors are consistently registered in the EMR, others are stored in patient episodes or in free text. GP’s know most of their patients well and are aware of the personal factors and circumstances of the patient. Sometimes, these personal factors are not registered, but are only remembered by the GP. GP’s value personal factors as being very important for continuity of care. Personal factors become more and more important in the set up of an individualized care plan. The process of incorporating patient context into care planning is known as patient centered or personalized care. It answers the question: ‘What is the best next thing for this patient at this time?’ Personal factors of patients are not consistently registered in the general practice. There is a lack of an easy accessible and neatly arranged view of patient contextual data in a lot of different EMR systems. Besides, there isn’t a commonly used standard list of personal factors among GP’s. Nowadays, more and more doctors work part time. The exchange and sharing of personal factor related information is necessary to contribute to patient centered care. Therefore the use of a classified set for registration of patient factors becomes important. 4 International Classification of Functioning, Disability and Health (ICF)
Personal factors can be described using an international classification of coding. It is possible to register different aspects of functioning of a patient using the International Classification of Functioning, Disability and Health (ICF). The last component of the ICF, Personal Factors (PF), is not classified because of the large social and cultural variance associated with them, but a definition for PF is given in the ICF. The setting in which personal factors of a patient are used, defines which personal factors are important to register. For example, a rehabilitation specialist might want to register other personal factor compared to a surgeon. Life events Leefstijlfactoren Persoonlijke werk gerelateerde factoren Niet onderzocht: Mentale factoren Ziekte en gezondheid gerelateerde factoren We will limit our research to develop a list of factors in 5 categories: General personal data, social context, lifestyle factors, life events and work related personal factors. These categories are derived from a proposal, developed by the ICF expert team of the Dutch Collaborating Centre and presented at the Network Meeting of the WHO in Brasil, This expert team based the proposal on an exploration of international articles. Categories such as psychological assets, mental factors and personal factors related to disease are not included, because it is still difficult to rate and describe factors from these categories. 8 Delphi studie Door het schema heen lopen. Rood: onderzoeksteam, geel: panelleden Benadrukken: 60 items dmv literatuurstudie. Uitnodiging gestuurd aan 54 huisartsen binnen TransHis en NMP. In Limesurvey vragenlijst opgesteld met duidelijk inleiding en voorbeeldvraag. In 1e ronde mogelijkheid nieuwe items toe te voegen. In eerste ronde antwoorden met ja, nee, soms. Na 2,5 week sluiting 1e ronde (2x herinnering gestuurd). Analyse van antwoorden dmv SPSS. Toelichtingen gegroepeerd. Geen toelichtingen uit de terugkoppeling gelaten. 2e vragenlijst gemaakt, met frequentietabellen en toelichtingen. Keuze uit ja en nee. Sluiting na 1,5 week 3e ronde: idem als 2e ronde. Resultaten 28 TransHis huisartsen  19 deelnemers 26 NMP huisartsen  11 deelnemers Na ronde 1: 26 deelnemers Gender Male 46% Female 54% Age 30-40 39% 40-50 15% 50+ We have invited 28 GPs using the TransHis EMR system for our study. 19 GPs agreed to participate, others were not able to participate due to time management issues. Our goal was to compose an expert panel of at least 20 members. Therefore, we also invited 26 GPs participating in the NMP. 11 GPs of the NMP agreed to join our study, resulting in a total of 30 participants. The invitation for the first questionnaire was sent to 30 GPs. In 2,5 week, after sending up to two reminders for our study, the first questionnaire was completed by 26 GPs. For the second and third round, participants were asked to complete the questionnaire within 1,5 week for each round. A reminder was sent if the questionnaire was not completed in one week. 10 Verloop Delphi studie Van de nieuwe items is er maar 1 geïncludeerd.
First round 26 of the invited 30 GPs completed the first questionnaire. A total of 652 clarifications were submitted, an average of 11 clarifications for each item. 25 new items were proposed. These new items were added to the round 2 questionnaire for evaluation by our expert panel. Using the 75% cut-off point, 15 items were included and 10 items were excluded for use in the definitive list. For the remainder 35 items, no consensus was achieved in round 1. These 35 items were again evaluated in the round 2 questionnaire. Second round All 26 members completed the second questionnaire. Using the 75% cut-off point, 4 items were included and 33 items were excluded. 498 clarifications were submitted, an average of 8 clarifications for each item. The remainder 23 items were used for our round 3 questionnaire. No new items were proposed in this round. Third and last round Again, all 26 members completed the third questionnaire. 1 item was included and 13 items were excluded. 178 clarifications were submitted, an average of 8 clarifications for each item. Consensus was not achieved for 9 items. The clarifications from round one, two and three were again summarized and reviewed for these 9 items by the study group. This process is described in the discussion. A total of 20 items are included in our final list of personal factors. Operationalisatie moeilijk: Analfabetisme: kan niet door patiënt zelf ingevuld worden (wel met hulp van d.a.) en hoe uit te vragen? Genetische factoren: open vraag te lastig voor patiënt om zelf in te vullen, mogelijk onjuiste informatie, maar zou wel kunnen met een keuze menu oid. Mogelijk geen eerlijke antwoorden: Analfabetisme kan schaamtegevoelens met zich meebrengen. Beter om in persoonlijk gesprek te vragen: Open vraag, andere life events en life events in jeugd Gebruik maken van objectieve gegevens: Overgewicht is niet geschikt om te vragen, beter om lengte en gewicht zelf te meten en eventueel een zorgepisode aan te maken als de patiënt hier zelf mee komt. 19 Discussie: sterke punten
Niet eerder dergelijke studie naar PF uitgevoerd in HA praktijk Delphi methode erg geschikt 26 panelleden hebben 3 ronden deelgenomen Hoge afkapwaarde van 75% Consensus bereikt bij veel items - Onderwerp waar nog niet veel over bekend is. (registratie van PF in huisartsenpraktijk) - Middels de Delphi methode worden toelichtingen van iedereen meegenomen en objectief beoordeeld in de volgende ronde, vanwege het anonieme karakter van de methode. Een vrij grote onderzoeksgroep van 26 huisartsen, die over het algemeen genomen goed overwogen toelichtingen bij alle items hebben gegeven. Geen uitval van proefpersonen tijdens de studie. Panel bestaat uit huisartsen die reeds veel registreren, veel ervaring hiermee hebben opgedaan, en daardoor kritisch kunnen oordelen welke items wel en niet geschikt zijn voor registratie. 20 Discussie: minder sterke punten
Nieuwe items niet voldoende toegelicht Niet altijd voor iedere patiënt Doel van huisartsen verschillend Operationalisatie? Keuzemogelijkheid ‘soms’ in ronde 1 - Sommige items, met name de nieuwe items, waren niet voldoende toegelicht waardoor het soms niet duidelijk was wat er mee bedoeld werd (zoals ‘familieomstandigheden’). - Sommige huisartsen hebben de eerste en tweede vragenlijst ingevuld zonder er rekening mee te houden dat het item voor iedere patiënt geregistreerd dient te worden. - Sommige huisartsen hebben de vragenlijsten ingevuld met in het achterhoofd de gedachte dat het doel van registratie van de items nodig is om onderzoek uit te voeren. - Het ontbreekt bij veel items aan een goede operationalisatie. 21 Toekomst Items verwerken in vragenlijst Vervolgonderzoeken:
Hoe worden patiënten gemotiveerd om de items zelf bij te werken? Welke items vinden patiënten belangrijk om in de huisartsenpraktijk te laten registreren? Hoe kan registratie van andere categorieën plaatsvinden, zoals ‘mentale factoren’ en ‘ziekte en gezondheid gerelateerde factoren’ - Vervolgstudie waar wordt onderzocht welke items patiënten belangrijk vinden om in de huisartsenpraktijk te laten registreren. - Voor een aantal items is het nog niet duidelijk hoe deze geoperationaliseerd moeten worden. Verder onderzoek is nodig. - Onderzoeken hoe patiënten gemotiveerd kunnen worden om de items zelf bij te werken. (app?)
