GGD is Dutch abbreviation for public health services. I will not ask you to use the name, but I will use it during my presentation, because I am going to talk about GGD MAKES SENSE. I will explain later what SENSE means

Vanwesenbeeck In 2005, a research study entitled “Sex under 25” was carried out in the Netherlands to investigate sexual health of young people between 12 and 25 years of age. Reason: Signals the sexual health of youth in the Netherlands was deteriorating: increase of STD (sexually transmitted diseases), increase of unsafe sex, increase of unwanted pregnancies. But recent research was lacking. The last big study date from 1995 So a study was needed to get reliable information about the sexual health of youth between 12 and 25 in the Netherlands. A research group of Rutgers Nisso (expert center for sexuality) and SOAIDS, (expert center for STD/HIV) De Graaf, Meijer, Poelman and Vanwesenbeeck,

Brede definitie, vooral afwezigheid van: SOA/HIV Ongewenste zwngerschap Dwang of gedwongen seks emotionele barriers, schaamte of gevoelens van schuld seksuele dysfuncties Vooral prettig respectvol seksleven! During the research they used a broad definition of sexual health, meaning The absence of STD/HIV The absence of unwanted pregnancies Absence of pressure, and absence of forced sex Absence of emotional barrier like feelings of guilt or shame Absence of sexual dysfunctions But most of all sexual health is about the capacity and possibility to enjoy sexuality in a respectfull envionment.

10 Zelfbeeld jongeren 1/ jongeren ( 39% (m) en 33% (v)) vindt zichzelf aantrekkelijk 1/3 jongens en 2/3 meisjes onzeker over eigen lichaam Kennis en interactie competenties. Seksuele identiteit

16 Signalen loverboy Meer interesse in seks.
Ziek zijn, spijbelen en weglopen. Nieuwe kleren, mobiele telefoon. Afzetten tegen ouders en school. Ziet er anders / uitdagender uit. Huilbuien en woedeaanvallen. Internet en GSM gebruik (sugababes, Partypeeps). Gaat veel op stap.

18 De 4 processen Grooming:
Ontmoeting, indruk maken, romantisch, relatievorming. Inlijving: Seksueel contact, isolement en afhankelijkheid, grenzen verleggen. Aanzet tot Prostitutie: Geweld, chantage, verslaving, controle, exploitatie. Beëindiging: Terugvalrisico, psychische problematiek, trauma.

20 Paula 16 jaar komt samen met haar vriendje (17)
Eerste vriendje sinds 6 maanden Vriendje heeft wel ander vriendin gehad. Paula gebruikt anticonceptiepil Paula komt voor SOA test, vriendje ook. Bij uitvragen pijn bij het vrijen.

Hulpvraag: veel jongeren ervaren vragen en problemen seksuele gezondheid gaan liever niet naar hun huisarts jongeren geven aan seksuele voorlichting belangrijk te vinden scholen vragen GGD om hulp, seksuele voorlichting is moeilijk

28 Een nieuwe start: Sense
VWS: Verbeteren seksuele hulpverlening Laagdrempelig voor jongeren Aanvullend aan huisarts Per 1 januari 2008 Samen met 8 SOA-Centra 8 Public Health Centra voor seksuele gezondheid Alleen poli 1 naam: Sense 1 website: Based on the results of the study “sex under 25” the Dutch ministry of Health concluded that improvement of sexual health care for youth was needed. They wanted a service for youth that was easily accessible. Youth that does not want to go to their general practitioner needed a place in first line health care they can turn to in case of sexual problems. In 2006 the public health services of the Netherlands implemented STD clinics, 8 in total. The ministry decided that the sexual health services should be combined with these clinics into Centres for Sexual Health So in 2008, eight centres for sexual health were set up and made responsible for sexual health care accessible and available for young people locally, all under 1 name, SENSE.

Seksueel geweld seksuele dysfuncties Verbeteren seksuele gezondheid Alleen individuele consulten - geen primaire preventie The key objectives of the MoH initiative included public health facilities such as offering advice on the prevention of unwanted pregnancies, STD, sexual violence and sexual dysfunctions. And off course the promotion of sexual health. To meet these objectives, the regional centres could do some sexual health consultations per year for youth under 25.

gratis voor jongeren<25 per hulpvraag 2 consulten per persoon per jaar hulpvragen: alles mbt seksuele gezondheid ruim 1700 consulten per jaar How did we organised in the region I am working: We started our training in January (first of the country) From the beginning we wanted a total integration of the sexual health care with our STD clinic. So we trained all our physicians and nurses. In this we differ from the other regions. Although the ministry of health wanted total integration in most regions this is not yet the case. In April opened the centre officially. This was an important moment to make the public and especially the youngsters aware of the centre We started with consultations, I will tell about later We are allowed to do 2400 consultations in 2008

31 Sense spreekuren Afspraakspreekuur (Heerlen, Geleen, Maastricht, Roermond, Venlo) Inloop spreekuren (gemeentehuis, JIP) Spreekuren op scholen Voorlichting Outreach We organised both walk in consultation slots and slots on appointment op places easily accessible for youth: for example city centre during shopping hours in the evening (after school time) and after the school holiday we started with consultations on some schools. At the moment these are still pilots The consultations at these school we combine with information/ health promotion meetings at school To promote Sense at the moment at some schools we organise only health promotion meetings, youth can make an appointment at our clinic. Outreach activities for example festivals like pinkpop

32 De praktijk Elke vraag over seksualiteit:
- Ik ben bang dat ik zwanger ben. - We willen seks hebben, hoe kan ik een zwangerschap voorkomen? - Mijn vriendje zet me onder druk om seks te hebben, maar ik wil nog niet ! - Ik heb pijn bij het vrijen. - Ik kom te snel klaar. - Hoe krijg je een SOA? - Ik vind mijn schaamlippen te groot/lang - mijn penis te klein. Leeftijd: 12-25 During the consultations we hear all possible questions about sexual health. For example: - I am afraid I am pregnant - We want to have sex, how can we prevent pregnancy? - My boyfriends forced me to have sex! - When I have sex it hurts - How can I get a STD? - I think my labia are too large / my penis is too short Untill now we see youth between the age of 12 till 25
